Want to start a business in Ballard City? Here's how:
Pre-Application Process:
- Verify that the business meets Ballard City Zoning requirements for the business address/ primary location by contacting the Ballard City Offices or the Planning and Zoning Division Chairman, Joe Allred.

Register your business with the state at corporations.utah.gov. This will register your business with the Utah State Tax Commission, Utah Department of Commerce and Department of Workforce Services. Licenses will be issued with the business name as it appears on the state website. Please follow all information and complete the information as requested.
You will need to bring a copy of the paperwork they send back with your DBA & the Tax ID# into the office with your completed application.

Get a copy of your state license (This is a copy of your Professional License, if applicable; see below for details).
If you have questions concerning your professional licensing, contact the office of the Utah Department of Professional Licensing at 1-866-275-3675.
- If you can say “yes” to any of the following, you will also need to register with the IRS and obtain an EIN.
- Do you have employees?
- Do you operate your business as a corporation or a partnership?
- Do you file any of these tax returns: Employment, Excise, or Alcohol, Tobacco and Firearms?
- Do you withhold taxes on income, other than wages, paid to a non-resident alien?
- Do you have a Keogh plan?
- Are you involved with any of the following types of organizations?
- Trusts, except certain grantor-owned revocable trusts, IRAs, Exempt Organization Business Income Tax Returns
- Estates
- Real estate mortgage investment conduits
- Non-profit organizations
- Farmers' cooperatives
- Plan administrators
- If your business provides any of the following, you will need a permit from Tricounty Health:
*Home businesses are not exempt from this requirement.- Pool/Spa on premises
- Barbering/Cosmetology Services
- Body Art / Tanning
- Food (Including Events and Food Trucks)
- Daycare/ Preschool/ In-Home Daycare
- Public Lodging
- RV/ Mobile Home Park
- Businesses selling foods under the Microenterprise Home Kitchen Act require a Tricounty health Permit.
- Cottage Food Establishments require a permit from the Utah Department of Agriculture.
- Obtain Copies of any additional Licenses/Permits your business may require
- Professional/ Occupational Licenses from the State of Utah
- Real Estate License
- Cosmetologist/Barber
- Master Esthetician
- Massage
- Nail Tech

- Professional/ Occupational Licenses from the State of Utah
Application Process:
- To apply for a Ballard Business License, you will need:
- Proof of registration with Utah State DOC
- State Tax ID Number
- EIN (if applicable)
- Health Department/Department of Agriculture Permits (if applicable)
- Professional/Occupational Licenses (if applicable)
- Business License Application, completed in full.
- Bring all applicable information to the Ballard City offices for review.
- Fill out application: Regular Business or Alcohol-Related Business as completely as possible.
- Fill out a Home Occupation Conditional Use Permit if required.
- Interview/Application Review: At this time we will determine if a Conditional Use Permit will need to be filled out. A Conditional Use Permit allows you to operate your business within the zone. A Conditional Use Permit will be required on the Home Occupation’s neighbors within 300 feet of your property will be notified of the intent of your business.
- PAY ALL FEES:
- New Business License Fee $100.00;
- Yearly Renewal $50.00 Beer License;
- Class A Off Premise Only $100.00;
- Class B Restaurant $150.00;
- Class C Lounge $200.00;
- Class D $200.00;
- Renewal Yearly $50.00.

- Inspection of Property/Place of Business: An Inspector may need to inspect location of business.
- Approval from all agencies will need to be obtained before license will be granted.
- Signatures from:
- Mayor,
- Sheriff Department,
- Building Inspector,
- Police Department,
- Health Department.
- Signatures from:
- Approval from the city council is required.
- Signatures from the City Council will need to be obtained. Business license will be submitted for approval at a regularly scheduled meeting for approval.
- Please fill out the Assessor's form as they will need to be informed of the business as well.
- Read this information and mail or fax attached form to the Assessor’s Office.
